Funding opportunity to Advance Health Equity

The National Association of County and City Health Officials (NACCHO) and the Association of State and Territorial Health Officials (ASTHO) released a request for proposals to fund up to 50 communities at $50,000 each to evaluate their work to advance health equity by addressing social determinants of health (SDoH).
The purpose of this funding opportunity is to identify and learn from multi-sector coalitions with a demonstrated history of addressing the SDoH outcomes related to chronic disease conditions, focusing on:
– Built environment – Clinical-community linkages – Food insecurity – Social connectedness – Tobacco-free policies – Physical activity illustration
The project will retrospectively evaluate and assess the impact of the implemented SDoH strategies and highlight successes and lessons learned. Local, state, tribal, and territorial health departments are encouraged to apply. Deadline: December 8, 2020 at 5 pm EST.
